Laser Biomodulation Therapy in the Treatment of Endometriosis

Endometriosis is the number one problem for women of the 21st century.
Endometriosis is a common gynecological disease in which cells similar to the endometrium (the inner layer of the uterus) grow outside this layer. In most cases, it develops in women of reproductive age, but cases of this disease have been described in girls (even before the onset of the first menstruation) and in older women, after menopause and cessation of menstruation.
Endometriosis affects women of reproductive age, its exact prevalence is unknown. Many women do not consult a doctor, considering pain to be a normal part of the menstrual cycle.
Endometriosis is one of the leading causes of pelvic pain and grounds for laparoscopic surgery in many countries. The average age of patients with endometriosis is 25-30 years. Endometriosis is less common in postmenopausal women. The causes of the disease have not been precisely established by official medicine.
There are many theories of endometriosis:
· theory of retrograde menstruation (Sampson);
· the theory of coelomic metaplasia;
· local biosynthesis of estrogens;
· inflammation;
· genetic aspects;
· embryological theory;
· the role of stem cells.
Classification
Endometriosis is divided into genital (internal and external) and extragenital.
Symptoms
1. Pelvic pain. Pain, as a rule, is cyclical (it appears or increases before menstruation), but it can be permanent.
2. Increased and prolonged menstrual bleeding.
3. Pain during sexual intercourse – dyspareunia.
4. Pain during bowel or bladder emptying (in the fourth stage).
5. Infertility.
6. A bloody tear (very rare).
Diagnosis of endometriosis
The "gold standard" is laparoscopy + histological verification of the diagnosis.
To correlate the diagnosis of endometriosis by laparoscopic visualization and the histological confirmation of the biopsy taken.
Frequently prescribed treatment: laparoscopy, hormone therapy.
It is important to pay attention to the fact that the ovaries lose their performance during interventions, this has been scientifically proven. Each laparoscopy reduces the chances of getting pregnant. Therefore, it is advisable not to resort to such an operation more than once in a lifetime.
That is why it is important to carry out further treatment with physiotherapeutic methods to preserve the functions of the ovaries (conservative therapy).
Conservative treatment is a type of medical treatment defined by the avoidance of invasive measures such as surgery, usually with the intent to preserve function.
There should be only one operation for endometriosis in a woman's life — the first and only one. For the purpose of anti-relapse therapy, medication may be prescribed according to indications. Studies in recent years indicate that currently drugs intended for the treatment of endometriosis do not adequately solve the problem. However, the progress in knowledge, especially in the field of laser therapy, observed in recent years, inspires confidence in the possibility of new therapeutic approaches.
A comprehensive examination and treatment of 169 patients with genital endometriosis was carried out, of which the main group consisted of 109 patients treated with vaginal laser therapy, and the control group - 60 patients receiving conventional treatment.
The developed patented method of laser therapy allowed to reduce the symptoms of the underlying disease in 81% of patients, vegetative dysfunction - in 67%, pathological changes in the psycho-emotional profile of the personality and other characteristics of the neuropsychiatric status – in 74% of patients, to reduce the frequency of relapses of the disease by 2 times compared with the control group who received conventional treatment (Table 1)*.

There are 4 stages of endometriosis , depending on the number of lesions and depth of infiltration.
With the 1st and 2nd degrees of severity of the disease, it is possible to avoid surgical intervention and use a physiotherapeutic method - laser therapy.
At 3-4 degrees of severity, laser therapy will help to remove invisible foci, restore menstruation and reproductive function. It helps to avoid complications after surgery.
